In some cases, you'll need to scroll down the page to find the correct form(s). In some cases, a nonprofit corporation is referred as a "nonstock" corporation.

In almost cases, you will need to have Adobe Acrobat Reader installed on your computer to download the forms. If the link to forms in your state changes (as they always do), we have also added the URL for the home page of the state to help you in your search. In addition, if you prefer to write or telephone, we have added those addresses and phone numbers as well.

We have also added Web sites and addresses for the offices that regulate nonprofit corporations in each state. Sometimes this is the Attorney General's office, and sometimes it is the same office that handles incorporations. Most states require nonprofit organizations to file an annual report, and many require registration to carry out fundraising activities. In most cases reporting requirements will be given to you when you file incorporation papers. If you do not receive it, please check with your state to see what reports and registration are required.
